This is real. Typically, the reason they do this is to enable the person to use their ankle as a knee. The will usually not attach that much of the leg to the thigh. They attach it so that the reversed ankle ends up at the same height as the other leg's knee. Then the person can slide on a fake leg and it usually provides them with a fairly decent gate. But in this case, I'm not sure why they would reverse the leg and leave it that long, unless they did it to try and preserve the foot for some future purpose. But anyway....this is legit. They actually do it on purpose, evidently for a variety of reasons.
